Access
to the
Provider Agency Registration
Step One: Appoint a
CDS Administrator for your agency
Step Two: Contact CDS at 877-353-2767. They will send you an Intent to
Participate Application via email and prepare your
Administrative Agreement from the information you supply. The CDS Administrative Fee in your agreement is based upon the number of individuals served by
your agency, according to the schedule below:
Fifteen or
fewer individuals served by agency: $1,800 annually
Sixteen to
thirty individuals served by agency: $2,400 annually
Greater than
30 individuals served by agency: $2,800 annually
The Administrative Agreement is for a three-year period with the
option to cancel in writing 60 days prior to each anniversary date of the
Agreement.
Step Three: Mail your signed agreement to:

Strategies, Inc.
Step Four: Elsevier,
the parent company for MC Strategies, Inc., will invoice you for the
administrative fee. The CDS implementation team will contact you when your
agency has been added to the site, usually within 2-3 business days. Your
Implementation Coordinator will work with you to set a convenient date and time
for your initial implementation.
Step One: Appoint a CDS point person for your county(ies)
Step Two: The
CDS point person should contact the technology support staff for the ODP
Consulting System. They will assist you
in completing the registration paperwork.

Registration paper work will be sent to the
Step Three: The
technology staff for the ODP Consulting System will contact you via email when
your county registration is complete, usually within 2-3 business days. At that point all people on your list should
have access to the CDS.
Registration for Individuals Receiving Support from ODP and Their
Families
Individuals and their families currently receiving support and
services through their local Mental Retardation Office can access CDS by having
the county register them or they can register themselves by contacting Sherri
Graven at: cdssupport@tiu11.org
There is no charge for individuals and their families currently
receiving support and services through their local Mental Retardation
Office.
